A Florida Pathway for UK-Trained GPs – How It Works

For UK-trained GPs who have considered practising medicine in the United States, Florida has recently become one of the most important states to watch.


Florida law now allows certain internationally trained physicians to obtain a medical licence without repeating a full U.S. residency, provided they meet a specific set of requirements. For experienced UK GPs, this creates a realistic pathway into U.S. clinical practice, although it remains a structured regulatory process rather than a shortcut.


Under Florida Statute 458.311, a graduate of a foreign medical school may qualify for licensure if they meet criteria including:


  • Holding an active, unencumbered licence to practise medicine in another country

  • Actively practising medicine for the four years immediately prior to applying

  • Completing postgraduate training substantially similar to an ACGME-accredited residency

  • Holding an active and valid ECFMG certificate

  • Having a full-time employment offer from a Florida healthcare provider


The Florida Board of Medicine ultimately determines whether the physician’s training is considered substantially equivalent.


What Is ECFMG and Why It Matters


ECFMG (Educational Commission for Foreign Medical Graduates) is the organisation responsible for verifying international medical credentials and examinations for physicians seeking to practise in the United States.


To obtain ECFMG certification, international physicians must typically complete:

  • USMLE examinations required by ECFMG

  • credential verification of their medical degree

  • a recognised pathway demonstrating clinical competence.


Many UK doctors qualify under ECFMG Pathway 1, which is designed for physicians who hold a licence or registration to practise medicine without supervision in another country.


Why Florida Created This Pathway


Florida introduced this licensing option in response to significant physician shortages and rapid population growth. The state has one of the fastest-growing and oldest populations in the United States, creating increasing demand for primary care physicians, geriatric specialists, and other clinicians.


Allowing experienced international physicians to enter the workforce more efficiently is one way Florida is addressing these workforce gaps.


Who This Pathway Is Best For


The Florida pathway is typically most suitable for:

  • UK-trained GPs with CCT or equivalent

  • Doctors listed on the GMC GP Register

  • Physicians with four or more years of independent clinical practice

  • Doctors who already hold ECFMG certification

  • Candidates with clean regulatory and employment records


Who This Pathway Is Not Usually Suitable For


This route is generally not suitable for:

  • doctors still in GP training

  • FY1 or FY2 doctors

  • doctors without ECFMG certification

  • doctors without recent clinical practice.

The pathway is best understood in a series of steps.


Typical journey:

ECFMG certification

Confirm four years of independent practice

Secure Florida employer
↓
Apply for Florida medical licence
↓
Background checks and Board review
↓
Licence issued
↓
Employer credentialing and start date

One important point is that this route is employment-linked. A physician must have a full-time job offer from a Florida healthcare provider before applying under this pathway.

Once licensed through this route, Florida law requires physicians to maintain employment with their sponsoring employer or another qualifying Florida healthcare provider for at least two consecutive years.


The Opportunity for UK GPs


For experienced GPs practising in the UK, this pathway may allow entry into U.S. clinical practice without repeating several years of U.S. residency training.


However, the process requires careful preparation. The Board must review:

  • postgraduate training equivalence

  • examination history

  • employment verification

  • background screening

  • regulatory history.


A well-prepared application with complete documentation is critical.
