Relocating to Canada as a UK-trained GP can open doors to a new way of working: more autonomy, less bureaucracy, and the potential for higher earnings. However, the clinic you choose makes a significant difference between a good move and a great one.
It’s not just about where you land geographically. Here’s what you should be looking at when comparing opportunities.
Pay Model and Overhead Split
Most Family Physicians in Canada work under either a Fee for Service (FFS) model or a capitated model such as the Family Health Organization (FHO). Each comes with pros and cons:
FFS rewards volume and works well in clinics with strong patient demand. However, pay attention to overhead splits and the support provided by the clinic.
FHO models offer stability, with payments based on enrolled patients and incentives for preventative care.
Make sure you understand which model suits your clinical style and financial goals. In addition, inquire about what is included in the overhead: admin support, EMR, nursing, billing, etc. It's worth noting that a higher overhead split (in your favor) isn’t always the best option. Having excellent support staff does come at a cost, but they help with more efficient billing and seeing more patients.
Onboarding Support
Relocating across the world is a major step. The best clinics recognize this and offer substantial support to help you get started. Look for clinics that provide:
Clear guidance on licensing (CPSO, CCFP, or other provincial routes)
Help with immigration, such as LMIA and work permit processing
A structured onboarding plan for your first few months
If a clinic offers none of this, ask how they’ve supported international hires in the past. A good onboarding process is essential for a smooth transition.
Patient Flow and Earning Potential
Some clinics may look promising on paper, but they lack the patient volume necessary for financial success. Ask direct questions like:
How many patients can I expect to see daily or weekly?
How are patients allocated among doctors?
What are new physicians earning in their first year?
Good clinics will have data and be open with you. Vague responses are a red flag. Understanding the flow of patients is crucial in making an informed decision.
Team Culture and Clinical Autonomy
Whether you’re joining a team of three or thirty, the internal culture significantly impacts your experience.
Will you have the freedom to practice your way?
Are there expectations regarding supervision or mentoring?
Is the clinic physician-owned or run by a corporate group?
For many UK GPs, the appeal of Canada lies in the opportunity to focus purely on clinical work, without layers of admin or performance targets. Ensure the clinic allows for that.
Long-Term Fit and Career Path
Do you want to stay long-term? Are you aiming for permanent residency? Could you see yourself becoming a partner or even opening your own clinic in the future?
The right clinic should support your goals. Look for:
Help with PR or longer-term immigration planning
Opportunities to buy-in or take on more responsibility over time
Flexibility with hours or scope of practice as your situation evolves
